Clinical referrals

The preferred route for clinical referrals is through SCI gateway. We urge you to use this electronic format as the preferred route for referral as this will enable prompt contact with patients.

For those that do not have access to SCI gateway a paper referral form can be completed electronically and sent securely via NHS mail to The paper form can either be found on Athena or can be obtained by contacting the hospice or by clicking here.

The referral form must be completed in full to allow us to provide an efficient and focused service to the patient. If appropriate, please ensure patients have had recent bloods taken.

The information received from the electronic referral will inform the hospice clinical team’s decision making. We will respond to the urgency of the referral and take full consideration of the patients identified needs. You will be asked if the patient is able to attend an outpatient appointment as well as to indicate which part of the hospice you feel the patient will benefit most from.

New referrals are discussed daily by the hospice MDT. All new referrals must be received by the hospice medical secretaries before 4pm on the previous day in order to be discussed at the morning meeting. The waiting list to prioritise in-patient admissions will also be discussed daily at this meeting.

Please note that all services are only for patients with life limiting illnesses however the family care service also offer support to family members of patients with life limiting illnesses. 

Patients can be referred to the hospice by any healthcare professional, providing the referral has been discussed with and agreed to by the patient.  Please ensure the patient’s GP and / or hospital Consultant is aware of the patient’s referral to the hospice.
