In your role you will provide an advisory and support service for patients, their families and their carers by working in collaboration with appropriate members of the multi-disciplinary team within primary and secondary health care, the voluntary sector and social services. You will direct and promote the specialist social work role within the Hospice and the high level interventions delivered to inpatients.
