Home Care A team of specialist nurses takes Hospice expertise to the patient and family at home.
Support to the family members and carers is an important part of the work of the Home Care team. All the nurses in the Home Care Team are registered nurses and have experience of working in the community. In addition they complete educational courses which qualify them as nurse specialists in palliative care. These nurses provide a valuable service to patients living at home by helping them cope with their illness, respecting their wishes with regard to their treatment and helping them remain at home whenever possible. The Home Care nurses maintain regular contact with the Hospice doctors and can offer expert advice on the relief of pain and other symptoms to both patients and their GP When attendance at Day Hospice or admission to the In-Patient Unit might be advantageous, the Home Care nurse will discuss this with the patient, family and GP to achieve the best possible support at all times.
