
Patients may be referred to the Hospice by any health care professional providing the referral has been discussed with and agreed to by the patient. The General Practitioner and/or Hospital Consultant must also be in agreement with this decision. The suitability of each referral is assessed by the multi-professional team. The preferred route of referral is in writing, using the Hospice specialist palliative care referral form.
Referral to the lymphoedema service is from the patient's general practitioner, hospital consultant, current lymphoedema practitioner or breast care nurse specialist. Each referral is assessed by a Hospice doctor and physiotherapist.
